Car Rentals:  When I last checked, all of the car rental agencies at the Lihue Airport offered hand controls on their vehicles free of charge as long as they had 48hrs notice.  Here's a list of the different car rental places: http://hawaii.gov/lih/ground-transportation/car-rentals

Van Rentals
:  Currently the only place to rent an accessible van on Kauai is through Wheelchair Getaways.  They can meet you at the airport or dock for your convenience and hand controls can be added at no extra charge.  Find out the specifics at their site :

Public Transportation: It costs $1.50 to ride The Kauai Bus from point to point. All buses on Kauai are lift equipped. Paratransit is available within 24 hours of notice M-F. Call The Kauai Bus Transportation Agency at 808-241-6410 or email thekauaibus@kauai.gov to make bus arrangements and for bus schedule information. You can also check their website Kauai.gov.

Group Tours:

- Both companies
have a wheelchair lift in their big buses to accommodate wheelchair passengers.  Be sure to call first and request the accessible bus for your specific trip.

Shuttle Service:

- The have a shuttle with a ramp to accommodate wheelchair passengers. Be sure to call 24-48hrs ahead of time to request the accessible shuttle.

Parking Placards: Visitors who have accessible parking placards from their respective states should bring them to Kauai. It is not possible to get an accessible parking placard once you are here. Bring your placard!

Portable Ramps: On Kauai there are no car rental agencies who have lift equipped vehicles. Gammie Home Care (808-632-2333 or gammie.com) rents portable ramps, wheelchairs, and other equipment. Some people rent a van and use a portable ramp to access the back seat, in some vans the entire middle seat can be removed to accommodate people who use motorized wheelchairs. Ask if an entire seat can be removed when you are talking with the car rental agencies.
