Car Rentals: When I last checked, all of the car rental agencies at the Lihue Airport offered hand controls on their vehicles free of charge as long as they had 48hrs notice. Here's a list of the different car rental places:

Parking Placards: Visitors who have accessible parking placards from
their respective states should bring them to Kauai. It is not possible
to get an accessible parking placard once you are here. Bring your
placard!
Portable Ramps: On Kauai there are no car rental agencies who have lift
equipped vehicles. Gammie Home Care (808-632-2333 or gammie.com) rents
portable ramps, wheelchairs, and other equipment. Some people rent a
van and use a portable ramp to access the back seat, in some vans the
entire middle seat can be removed to accommodate people who use
motorized wheelchairs. Ask if an entire seat can be removed when you
are talking with the car rental agencies.
